*HEAD COACH & PARENT authorization is required prior to the evaluation. Click Here for the authorization form. No jump/spin technical suggestions or comments are included in this evaluation. Evaluation is first shared with the coach, and then passed onto the figure skater.

Fees:
$35/program
$60/short & long program combo

How It Works:
Film the figure skating program in its entirety from the side of the rink      showing the judges’ perspective.
     - Figure skating programs performed with or without costume are accepted.
     - Professional Footage Preferred

 Send a notification e-mail with name and level of the skater along with      the completed Creativeskating.com: Authorization of Choreography      Evaluation to ehs@creativeskating.com

Please send the skating program in DVD format along with a check or      money order made out to Elizabeth Hollett Shackett.
     Note: If video must be returned there will be an additional fee of $15
